'Upload OSM Data' tool UI needs adjustment

When the Upload OSM Data tool is opened in Desktop, the UI has some display issues. There are 2, see attachment:
1) The 'User' and 'Password' parameters are partially cut off.
2) The revision table help instructions (put cursor on revision table field and click "Show Help" button at bottom) has a typo in the revision table name.
